We have a team of home carers specialising in providing end of life care to customers in their own home. Our team will visit you to create an advanced Care plan specifically detailing your wishes and the things that are important to you and your care at this crucial time in your life. The plan will go into detail about what makes your life meaningful, the things you want your family and friends to remember at this time and what is important to you.

 

Our support can include help with personal care: pain management, medication support, cooking meals and companionship and we will provide a care plan to the rapidly changing needs of our clients.

Palliative care

From our experience we know that many people wish to remain at home with their loved ones to spend their last days their families. It can also be very difficult to take care of a loved one who is at the end of their life. 

Our team will match a small team of care workers that will build meaningful relationship with you and your support network, This ensure continuity allowing us to monitor changes on day to day basis for you to feel at ease with regular friendly faces.

We also work closely with district nurses and community health care team to ensure our customers are treated with compassion, dignity and respect at all times.
